Responsibilities

  • Document patient encounters from start to finish, including patient history, physician exam findings, provider orders, and patient outcomes
  • Follow providers into patient rooms and serve as the real-time documentor during provider-patient interactions
  • Transcribe patient appointments and chart medical data discussed during clinical visits
  • Input medical records and information into the Electronic Health Record (EHR) system accurately and efficiently
  • Document History of Present Illness (HPI) based on observed provider-patient interactions
  • Record laboratory results, imaging studies, and diagnostic test information in patient charts
  • Listen to medical dictations from physicians and translate them into written documentation in the EHR
  • Organize patient information in the EHR prior to each patient encounter to enable efficient workflow
  • Review and edit drafts prepared by speech recognition software for accuracy
  • Translate medical abbreviations and jargon into appropriate long form documentation
  • Write referral letters for physicians and prepare other types of medical correspondence
  • Facilitate diagnostic tests and referrals to medical specialists as directed by providers
  • Follow up with healthcare providers to ensure reports and documentation are accurate and complete
  • Manage an average of at least two patients per hour, and a minimum of four patients per hour during peak times
  • Maintain a clean and professional working environment
  • Assist with training of newly hired Medical Scribes
  • Adhere to all HIPAA regulations, confidentiality requirements, and patient rights standards

  • High school diploma required; Bachelor's degree or equivalent completion within one year of start date
  • Knowledge of medical terminology
