  • Oversee admission and discharge activities, including developing discharge plans and coordinating post-discharge requirements

  • Prepare reports by collecting, analyzing, and summarizing treatment data, outcomes, and trends
  • Educate clients and families about their rights, available resources, treatment plans, and self-management strategies
  • Coordinate with insurance companies, Medicare, Medicaid, and other reimbursement sources
  • Facilitate referrals to specialists, mental health programs, substance abuse services, and other appropriate providers

Required Skills & Qualifications

  • Bachelor's degree in Social Work, Nursing, Psychology, Human Services, Healthcare Administration, or a related field
  • Minimum 2-3 years of experience in case management, social services, or healthcare coordination
  • Strong verbal and written communication skills with the ability to explain complex information clearly
  • Excellent organizational and time management abilities to handle multiple cases simultaneously
  • Demonstrated empathy and compassion when working with diverse populations
  • Strong problem-solving and critical thinking skills
  • Ability to work independently and as part of an interdisciplinary team
